I find it amazing that the said carriage was put into service.

Surely someone in management of the South Devon Rly should have refused to let the carriage go into service.

Three investigations - SDR, ORR and RAIB are underway. The SDR has made a statement, kudos to them for that and accepting something has gone wrong. I'm not sure what can be added without speculating, other than to be thankful there was no injury.

To the extent it is relevant on an SVR forum, I am sure the SVR operating team are double checking SVR procedures and will issue reminders as necessary. I am very thankful for the staff and volunteers that do this.

FWIW I wish all - the mother and child and the SDR - well.

None of us can be complacent when it comes to safety, and we must learn from other's misfortunes.
